Instructions for Accessing ACHQC Data for Research
- Data collected for quality improvement purposes in the ACHQC can be used for research in certain circumstances
- Any use of ACHQC data for research purposes should involve either your local or private Institutional Review Board (IRB)
- ACHQC surgeons and individually approved personnel can access your own patients’ data any time for research by direct download from the ACHQC user interface. Please be aware that downloading information in this manner may include Patient Health Information (PHI) depending on your selections for export. Please follow your local institutional or practice policies regarding the proper handling of PHI
- ACHQC surgeons can request a summary analyzed research data for all ACHQC patients by completing the appropriate form below. The request is approved by the ACHQC Data Use and Publications Committee and analysis performed in partnership between the requesting surgeon or group and the ACHQC Data Coordination Center. Currently, this service is provided free of charge to ACHQC surgeons in good standing
- Members of industry and researchers can also request summary analyzed research data by completing the appropriate form below. Individual fees may apply
- ACHQC Foundation does not license ACHQC data to any person, including, without limitation, to participating surgeons and institutions and Foundation Partners, for use in litigation

The ACHQC recognizes that it can provide useful information for a wide variety of uses that fit the mission of the collaborative and that some requests may not originate from ACHQC surgeons or Foundation Partners. Typical stakeholders in this scenario would include medical researchers, governmental agencies, or companies that are not currently ACHQC Foundation Partners. UNDER NO CIRCUMSTANCE, HOWEVER, WILL ACHQC FOUNDATION HONOR ANY REQUEST FOR THE USE OF ACHQC DATA IN LITIGATION OR PROCESSES RELATED TO LITIGATION.
