Abdominal Core Health Quality Collaborative Foundation (“ACHQC Foundation,", “we,” or “us”) is committed to protecting the privacy of its members and visitors with respect to the personal information they provide us through our website at https://achqc.org/ or the mobile application through which you initiated access to this Privacy Policy (either of which his referred to as an “Electronic Site”).

By visiting an ACHQC Foundation Electronic Site you are accepting the practices described in this Privacy Policy (“Policy”) with respect to information that ACHQC Foundation collects about you through its Electronic Sites.

ACHQC Foundation may amend this Policy at any time without notice.  Changes to this Policy may affect our use of personally identifiable information that you provided us prior to such changes.  Please visit any of our Electronic Sites to obtain current information about our Policy.

ACHQC Foundation uses various technologies to protect the confidentiality, security, and integrity of information volunteered or collected from users of the Electronic Sites. We do not require anyone to divulge any personally identifiable information to use the Electronic Sites, although the use of certain features may require that you provide us with personally identifiable information to use those features. ACHQC Foundation will not collect personally identifiable information about you unless the information is provided voluntarily by you.

ACHQC Foundation does not knowingly collect, use or disclose personally identifiable information about any patient on its Electronic Sites. You should not post any material in any area of our Electronic Sites that could be used to identify a patient. If we learn that we have collected the personally identifiable information of a patient, we will take steps to delete the information as soon as possible.

You must be at least 18 years old to have our permission to use this site. We do not knowingly collect, use or disclose personally identifiable information about visitors under 18 years of age.

I. Information ACHQC Foundation Collects

The information we collect generally includes: (1) personal information you supply and (2) tracking information collected as you navigate through the Electronic Sites.

Information Provided by You:

We may collect and store personally identifiable information that you provide to us.  The personally identifiable information we request from ACHQC members and their staff who have or whose employer has a separate agreement with ACHQC Foundation to access and input certain data through the Electronic Sites may include: (i) first and last name; (ii) email address; (iii) phone numbers, and (iv) your employer’s name. As of the date of this Policy, ACHQC Foundation does not collect payments through its Electronic Sites.

The ORACLE predictive tool that is available through the Electronic Sites does not require or request any personal identifying information although its use requires the input of certain non-identifying health information to produce a predictive result, but the Electronic Sites do not save or store that information.

In addition to the foregoing, members of the ACHQC collaborative may elect to submit requests to obtain and publish data provided by ACHQC; these requests may ask for identifying information and the nature of your professional work.

Information Collected Through Technology: 
Primarily to tailor the Electronic Sites to your needs, ACHQC Foundation collects information using technology, such as IP addresses and cookies. For example, your IP address provides ACHQC Foundation with non-personally identifiable information about how you entered the Internet, namely through your Internet service provider, your company, or some other source, and can permit ACHQC Foundation to identify the frequency with which you visit the Electronic Sites. Standard features in Internet browser software such as "cookies" allow the Electronic Sites to track certain user activities by assigning each of our visitors a unique number that resides on your computer. ACHQC Foundation uses cookies to make the Electronic Sites more interesting and useful (for example, cookies let us know which of our sites you have been visiting and which you have not).

II. How ACHQC Foundation Uses Information It Collects

ACHQC Foundation uses your personally identifiable information for the purposes for which it was submitted.  For example, ACHQC Foundation uses the information collected to enable ACHQC Foundation to verify the identity of a user and to create and maintain an account for users to input and access data in the ACHQC registry. We do not undertake to maintain any information about you for a duration greater than necessary to provide services to you or as provided by law, rule or regulation.

Once someone voluntarily provides an email address, ACHQC Foundation assumes it has permission to email that person with questions, transaction follow-up, membership information, and to send bulk email messages. ACHQC Foundation may collect data to track the effectiveness of emails to enable it to better serve its audiences. ACHQC Foundation does not sell or lease the email addresses of its users.

ACHQC Foundation may use information about use of the Electronic Sites and aggregated information about you and other visitors to our Electronic Sites in order to improve our Electronic Sites and develop content and services that may be of interest to you and for its other business purposes.

Users with an account who log into an Electronic Site will not be anonymous in the non-public areas of that Electronic Site and may not always be anonymous in the public areas of the Electronic Site. Users without an account will be anonymous in their browsing of the Electronic Sites, except to the extent they voluntarily submit personally identifiable information through the Electronic Sites.

III. Protection of Personally Identifiable Information

ACHQC Foundation employs reasonable security measures to protect your personal information, but please be aware that no method of electronic transmission is completely infallible, and we cannot and do guarantee the absolute security of your personal information.

IV. Information Sharing with Third Parties

We may share your personal information with our authorized service providers that perform certain services on our behalf.  These services may include providing customer service and supporting our Electronic Sites’ functionality.  These service providers may have access to personally identifiable information needed to perform their functions but are not authorized to share or use such information for any other purposes. 

We may also provide your information to third parties to accomplish the purpose for which the information was submitted.

We may also transfer user or customer information, including personally identifiable information, in connection with a corporate transaction involving us, such as a divestiture, merger, consolidation, or asset sale.

We may disclose specific information in response to a valid court order, subpoena, government investigation, or as otherwise required by law, to enforce our Electronic Site policies, or to protect our or others' rights, property, or safety.  We reserve the right to report to law enforcement agencies any activities that we, in good faith, believe to be unlawful.  We may share information to prevent illegal uses of the Electronic Sites or defend ourselves against third-party claims.  We may also share information with companies assisting in fraud protection or investigation.  We do not provide information to these agencies or companies for marketing or commercial purposes.

This Policy is subject to, a part of, and incorporated into the Terms of Use which can be found under the Terms of Use tab on the Home Page of the Website and on other Electronic Sites.

The Electronic Sites are operated from the United States. You consent to your personally identifiable information being transmitted to and processed within the United States according to this Policy regardless of your residency, citizenship, or location when you input such data and so long as the data is maintained by ACHQC Foundation. 

The Electronic Sites may also contain links to third party websites that are not under our control. You may wish to consider that these websites may have their own privacy policies or no privacy policies at all. ACHQC Foundation is not responsible for the privacy practices used by these linked websites.
